Release Forms

These forms are required to grant temporary or permanent release privileges to persons not listed on your child(ren)'s registration form(s). They may also be used to remove individuals from your child(ren)'s permanent release list(s) or to inform Extended Care that your child(ren) will not be attending on a regularly scheduled day.
